NHS Confederation share lessons learned on improving the patient journey
The NHS Confederation has unveiled Improvement across the interface: Lessons from collaboratives, a new guide showcasing practical, real-world strategies to bridge gaps between primary and secondary care, where patients often experience fragmentation or delays in their journey.
Drawing on insights from an eight-month collaborative involving 122 healthcare professionals across 11 cross-system teams, the resource provides a rich array of advice, examples, and recommendations to support seamless and safe transition of patient care.

Understanding the problem: Teams emphasised the importance of dedicating time early for different professionals to align on what the shared challenge truly is

Building relationships: Trust and mutual understanding were foundational, strategies like informal gatherings, shadowing, and stakeholder mapping underpinned long-term engagement

Communication & connection: Mixing virtual sessions, face-to-face meetings, written guides, and “buddy” peer support maintained cohesion and momentum across systems

Leadership, governance & data-led insights: Sustained improvement relied on senior involvement, shared leadership practices, governance mechanisms, and robust data to both inform and sustain change, alongside securing necessary investment.
