Radiotherapy errors – latest report published

Public Health England has published the latest report on radiotherapy errors and near misses, covering the period Dec 2011-Nov 2013

The report analyses the 7742 Radiotherapy Error and Near Miss (RTE) reports submitted from Dec 2011 to Nov 2013, from a total of 413,730 radiotherapy prescriptions delivered during this period – an RTE rate of 1.9%.

The most common categories of RTEs were identified as:

  • accuracy of data entry
  • on-set imaging: production process
  • on-set imaging: approval process
  • use of on-set imaging
  • documentation of instructions/information
  • on-set imaging: recording process
  • recording of patient specific instructions
  • movements from reference marks
  • consent process
  • generation of plan for approval

The report shows a continuing downward trend in the number of reportable and non-reportable radiation incidents, while the level of overall reporting continues to increase.
