Opportunity for delivering value based and population healthcare

An opportunity to develop skills in value based and population healthcare for Healthcare Scientists who deliver services to patients presenting with symptoms of breathlessness, dizziness or abdominal pain.

There are 20 places available on this programme, which has no charge although successful participants will have to meet their own travelling and subsistence costs and any backfill charges.  The attached flyer gives more information with an application form.  The closing date for applications is Friday 6 November 2015.

The programme supports the vision of the Five Year Forward View by providing training to systematically develop skills in value based and population healthcare.  As part of the programme, participants will complete a project demonstrating the implementation of innovative models of care across their service.
